What this website collects
Almost nothing. This page loads no analytics, no advertising pixels, no
third-party scripts, and no tracking cookies. The only thing it stores on your device is
your color-theme preference. If you submit the info-visit form, we receive the name,
contact detail, and preferred time you typed — nothing else.
How form information is used
Solely to contact you and schedule your info visit. It is stored on the
practice's own self-hosted systems — never sold, never shared with third parties, and
never added to a mailing list. To have it deleted, email
info@excelsiormedicine.com and we will remove it.
Health information
This website is not for medical questions and does not collect health
information — please don't include medical details in the form. Patient records live in
the practice's own electronic health record system, protected under HIPAA, entirely
separate from this website. The portal screenshots on this site show a made-up
patient, never a real one.
