What is direct primary care?
Direct primary care (DPC) means you pay the practice directly — a flat $250
per month — instead of us billing insurance. No claims, no copays, no coding games. That is
what buys unhurried visits, preventive care that actually gets done, and real availability.
It is not insurance; keep yours for hospitals, specialists, and emergencies.

Is direct primary care worth it if I'm healthy?
Prevention is where an unhurried physician pays off most: the screening that
gets scheduled, the blood pressure that gets caught early, the strength you keep. Healthy
members use the portal, the calendar, and the assistant most of the year and the physician
when it counts.
Does this replace my insurance?
No. Your membership covers your primary care. Insurance still matters for
hospitals, specialists, emergencies, imaging, and prescriptions. Many members pair the
membership with a high-deductible plan.
How is this different from concierge medicine?
Concierge practices usually charge a retainer and bill your insurance.
Direct primary care charges one flat fee and bills nobody. Same unhurried access, simpler
money — and here, the technology behind it is built in-house rather than rented.
What does membership cost?
$250 per month, paid monthly. Flat — no enrollment fee, no copays, cancel
anytime. It buys unhurried visits with your physician and the availability to actually use
them.
Can I pay with my HSA?
Federal rules changed in 2026 to allow HSA reimbursement of direct primary care
fees up to a monthly limit that sits below this membership's price. Whether your
administrator reimburses part of it is up to your plan — ask them, and we'll provide the
documentation.
What's in the patient portal?
Your records, visits, medications and prevention plan; a health log your watch,
cuff, and scale can feed; a calendar and to-dos that sync to your phone; documents; private
video visits; messages with your care team; and the Excelsior assistant — it answers, logs
your numbers when you tell it to, takes your story before each visit, and takes voice.
Everything it writes is reviewed by your physician. All of it self-hosted by the practice.

Is the assistant making medical decisions?
No. It organizes — it takes your story, logs what you tell it, explains your
medications from published sources, and reminds you what's due. Nothing it writes reaches
your chart without your physician reviewing it first.
Where does my data live?
On the practice's own servers, in a records system your physician built and
runs. Encrypted, HIPAA-secure, with every portal read written to your chart's audit trail.
Never sold, never mined, never shared with an ad network. This website loads zero
trackers.
Do you prescribe pain or psychiatric medications?
Controlled pain medications and psychiatric medications are best managed by the
clinicians who specialize in them, so we refer those out. If they are central to your care, we
will say so at the info visit and help you find that care.
How does the weight-management program work?
It starts with an unhurried evaluation: history, labs, body composition,
training and nutrition review. From there we build the plan together — and when modern
weight-loss medications are clinically appropriate, we prescribe and actually follow them
properly, with labs and visits, not a monthly auto-refill.
Why does a doctor's office have a movement lab?
Because how you move predicts how you age. Watching you squat, hinge, and walk
tells your physician more than a questionnaire, and gives a real baseline to train against.
The lab opens with the office; equipment, possibly a rack and a treadmill, is planned.
